COVID-19 VACCINE

Our practice has been selected to participate in the National COVID-19 Vaccination Program.

Covid-19 Vaccine

You are now able to Book an Appointment with us at the Corowa Medical Centre for your COVID Vaccine Dose 1. We are an approved vaccination provider for the Covid-19 vaccines and are happy to help provide the immunisation for our local area and broader community.

We will be holding regular vaccine clinics, and if you would like to know if you are eligible for the dose 1 vaccine, you can visit the Covid Vaccine Eligibilty Checker website or you may check with us if booking via phone.

If using the Covid Vaccine Eligibilty Checker this will tell you which phase of the rollout you are in. If you are not yet eligible, you will be able to register your interest so that you can be notified when you are able to book.

Booking Your COVID-19 Vaccination Appointment

When it is your turn to receive your dose 1 vaccination, you can book online or call us on 02 6030 5500 to book your appointment.

If you have had another vaccine in the 14 days before your COVID-19 vaccine appointment, please let us know as your vaccinations may need to be 14 days apart.

Getting ready for your appointment

Please do not come to your vaccination appointment if:

  • you are feeling unwell with fever, cough, runny nose or other symptoms that could be from COVID-19;
  • you are waiting for COVID-19 test results, or have tested positive for COVID-19;
  • you are a close contact of someone with COVID-19, or;
  • you are in quarantine.

If you cannot make it to your vaccination appointment, please contact us to arrange a new appointment.

Covid-19 Vaccine Consent Form

Consent

All patients MUST complete a Vaccination Consent Form prior to be vaccinated against COVID-19.

Please fill out the Consent Form prior to coming in. Alternatively, our reception staff are able to provide you with a hard copy.

Once submitted, our staff will decide if it is appropriate to book you in to one of our dedicated vaccination clinics.  If we decide your situation needs to be reviewed, or if you need more information before you feel confident making an informed decision, you will need to book a regular appointment to discuss.
